NON-EMERGENCY TREATMENT IN AN EEA COUNTRY

If you need continuing treatment for a pre-existing medical condition, or if you travel to an EEA country specifically for medical care, then Form E112 is required.

Application for E112 in England and Wales should be made to:-

Department of Health, International Branch

In Scotland and Northern Ireland application should be made to the appropriate Health Board.
